If you have multiple family members who wish to begin at the same time, please reach out for scheduling as multi-family appointments are discounted.

For anyone not established at this office looking for chiropractic care. Every new patient adult/child is required to complete a new patient exam with the doctor. The initial assessment includes a health history, vitals, orthopedic exams, and other patient specific exams. This is required to determine if each patient is a candidate for chiropractic care. If cleared for care you or your child will be adjusted at this appointment. Please bring any recent imaging or other tests pertinent to your care.

Chiropractic Follow Up Visit

15 minute follow-up adjustment for established patients only. This is for patients that have already had their new patient evaluation. If booking for 4+ people please call us for scheduling help at (689)204-3057 or hello@happydayschiropractic.com

Acupuncture New Patient Initial Visit

Acupuncture first visit. To establish a history and complete patient questionnaire and physical exam. First session included. If you are an established patient, talk to your doctor at your next appointment about adding acupuncture to your care plan.

Acupuncture Follow-Up Visit

Once you have had a new patient evaluation and have been cleared for acupuncture you may schedule a follow up session. A questionnaire will be given to determine if your condition is acute or chronic, and a proposed treatment plan will follow.

CFT/ Gillespie Approach Initial Visit

Initial visit and new patient exam for CFT. If you are an established patient being treated at this office and have already had a new patient exam for chiropractic or acupuncture, you will only need a small amount of additional information recorded at your appointment and can book the CFT follow up visit instead. This slot is for chronic patients that have been dealing with their issues for a long time. Typically the longer something has been an issue, the harder it fights to stay around. School Physical

State required Florida school physical. If your school requires forms differing from the state wide school physical forms please bring them with you. If your child only needs the standardized state school physical form we will provide that for you. Dr. Hope was an avid sports kid with a single mom so this school physical is discounted to help others that find themselves in a similar situation.

Re-Evaluation

This is for those of you who were established/regular patients at Happy Days Chiropractic previously, and are ready to reprioritize your nervous system well-being and resume chiropractic care after more than 6 months of time, or if you have a new injury or life event that you would like help with.
